Verification Overview
The purpose of this site is to expedite the process of verifying the status of a physician's medical license. It is intended for use by physicians who hold one or more medical licenses in the United States and need to have verification of their licensure status sent to another state medical or osteopathic board.
A licensure verification statement can be sent to verify the status of a license issued by any of the participating state licensure boards. You will still need to contact non-participating state boards individually to have a verification statement sent from those boards. A verification statement can be sent from VeriDoc to any medical or osteopathic licensing board in the U.S.
Sending verifications from this site is a simple four step process.
Step 1: You will be asked to enter identifying information about yourself which will then be used to locate your license information by searching through all licenses issued by participating state licensure boards.
Step 2: Information for each license found will be displayed. Here you will select the entries that you wish to include in your verification statement(s).
Note: Most state boards generally require verification of every medical license you have ever held whether or not the license is active at this time.
Step 3: You will be asked to select the state board to whom the verification statements should be sent. A verification can be sent for each participating state board from whom you have ever been granted a license. Different fees may be charged by each State board.
Step 4: Here you will be asked to enter your credit card information. After the credit card has been accepted a receipt showing each verification sent will be generated for you to view and print.
